First Day:

- If you have Dr. Sharma at Down State Medical Center, your start time is 10 AM

- Dr. Maltz-  9:30 AM  main pharmacy

- Bellevue- 8:00 AM volunteer office, you will most likely be given more info regarding procedures for this site by the volunteer office

- Anywhere else start time is 9:30 AM

Bring the following:

-complete copy of health form (this should be identical to what you gave Ms. Durani)

- Proof of HIPPA training

- Original intern permit

- Lab coat, name tag, 2 forms of picture ID

-Remember to ask your preceptor about healthform and original intern permit on the first day
